Structured Support. Personal Guidance. Real Outcomes.
At Homes2Care, we believe that meaningful change begins with stability — and that stability starts with the right level of support. Our residents come from many walks of life: some are young people leaving care for the first time; others are adults rebuilding after crisis, trauma, or homelessness. Each person's story is different, so our care must be too.
That's why we use a Tier 1–4 support model — a flexible, structured framework that tailors housing and care to the individual. It ensures that everyone receives exactly the right level of attention, structure, and guidance for their needs — and can move smoothly between tiers as their situation improves.
The Homes2Care tiered system isn't just about care levels — it's about pathways to progress. It ensures no one receives too little support to cope or too much to grow dependent. By scaling our input based on individual needs, we help residents gain control of their lives at a sustainable pace.
Residents can progress from intensive to light-touch support as their confidence grows. Our flexible model allows smooth transitions between tiers, ensuring consistent support throughout their journey to independence.
Councils, commissioners, and families can see clear evidence of support activity and outcomes. Our structured approach provides transparent reporting and measurable progress indicators at every tier.
Support packages can adapt in real time to changing needs or risks. Whether a resident needs additional help during a challenging period or is ready to reduce support, we respond quickly and appropriately.
Residents gain life skills, self-awareness, and the confidence to live independently. Our tiered approach builds capability gradually, ensuring sustainable progress rather than overwhelming residents with expectations.
"Our model allows residents to gain control of their lives at a sustainable pace, moving from dependency to independence with confidence and dignity."
Early Prevention and Independence Building
Tier 1 focuses on early intervention — preventing homelessness, disengagement, or crisis before it happens. It's designed for young adults and individuals with low-level needs who are capable of independent living but still benefit from structured guidance and regular check-ins.
Residents in Tier 1 receive 1–2 structured visits per week, often through floating support. Advisors help maintain tenancy responsibilities, manage bills, and encourage personal organisation.
The aim is to help residents sustain independence while knowing that help is available when needed.
Property care and rental responsibilities
Money management and routine building
Accessing training and work opportunities
Developing confidence and self-management
Early detection before issues escalate
Residents in Tier 1 typically progress toward independent tenancies or move into employment and training programmes with minimal ongoing support — achieving genuine autonomy.
Structure, Guidance, and Stability
Tier 2 is for individuals who require more frequent and structured engagement. Many in this tier are stabilising after crisis or developing the tools needed to transition from supported living to full independence.
Residents receive 3–4 structured sessions per week with a dedicated keyworker. These sessions focus on strengthening personal routines, emotional wellbeing, and goal setting.
Advisors help residents create realistic plans around training, employment, and community integration.
Weekly progress reviews and achievement tracking
Coping mechanisms and confidence building
Shopping, cooking, and time management
Building positive relationships and connections
Job applications, CV writing, volunteering
College enrolment and training courses
Tier 2 support enables residents to stabilise emotionally, manage their lives more independently, and reduce reliance on statutory services, creating a firm platform for success.
Intensive Care and Risk Management
Tier 3 is designed for residents with high or complex personal needs that require consistent, hands-on support. This tier provides a safe, structured environment where individuals can rebuild trust, manage risk, and regain control.
Residents receive daily structured contact, combining intensive keywork with coordinated multi-agency involvement (social care, probation, NHS, etc.).
Staff ensure a stable daily routine while addressing both practical and emotional barriers to recovery.
Structured sessions with outcome tracking
Risk reduction and safety planning
Health, probation, and social care coordination
Self-care and wellbeing strategies
Developing structure and accountability
24-Hour, Specialist Care
Tier 4 is the most intensive level of support within the Homes2Care model — providing specialist, round-the-clock assistance for residents with significant or clinical needs. It focuses on safety, dignity, and long-term recovery.
Tier 4 properties are staffed 24 hours a day, with dedicated 1:1 or 2:1 support teams working under clinical supervision.
Each care plan is bespoke, designed around medical input, psychological support, and daily living assistance.
Structured daily routines with constant support
Liaison with psychiatrists and psychologists
Health coordination and monitoring
Complex safeguarding protocols
Building sense of personal agency
Dedicated support staff provide individualized attention, ensuring safety, dignity, and personalized care at all times.
A Bridge Between Care and Independence
Every young adult within Homes2Care has access to a Personal Advisor (PA) — a trusted, consistent professional who helps them plan their future and navigate adult life.
For many care leavers and young people, the PA becomes the most stable adult relationship in their lives. They act as a mentor, advocate, and coach, guiding residents through housing, education, and personal challenges with compassion and consistency.
"A Personal Advisor creates continuity, belonging, and hope for young adults who have experienced instability, trauma, or time in care."
Every resident has their own Personal Advisor
Building trust and offering someone to talk to about goals, fears, and decisions
Helping with budgeting, job applications, CV writing, and tenancy management
Linking young adults to education, training, or mental health services
Offering ongoing, familiar support — especially during times of transition or instability
Ensuring young people's voices are heard in care planning and life decisions
For young adults who have experienced instability, trauma, or time in care, having a Personal Advisor creates continuity, belonging, and hope. PAs help them visualise a future beyond supported housing — connecting them to work, study, or community life.
At Homes2Care, our support system isn't about accommodation alone — it's about transformation. Each tier builds toward independence, and every advisor helps residents turn goals into achievements.
From structured support to self-sufficiency, from dependency to direction — we create homes that heal, empower, and prepare people for brighter futures.
