That's why we use a Tier 1–4 support model — a flexible, structured framework that tailors housing and care to the individual. It ensures that everyone receives exactly the right level of attention, structure, and guidance for their needs — and can move smoothly between tiers as their situation improves.

Tier 1

Low Support

Early Prevention and Independence Building

Tier 1 focuses on early intervention — preventing homelessness, disengagement, or crisis before it happens. It's designed for young adults and individuals with low-level needs who are capable of independent living but still benefit from structured guidance and regular check-ins.

Typical Residents

  • Care leavers aged 18–25 transitioning into independence
  • NEET (Not in Education, Employment, or Training) young people
  • Adults at low risk of homelessness or social isolation

Support Approach

Residents in Tier 1 receive 1–2 structured visits per week, often through floating support. Advisors help maintain tenancy responsibilities, manage bills, and encourage personal organisation.

The aim is to help residents sustain independence while knowing that help is available when needed.

Tier 2

Medium Support

Structure, Guidance, and Stability

Tier 2 is for individuals who require more frequent and structured engagement. Many in this tier are stabilising after crisis or developing the tools needed to transition from supported living to full independence.

Typical Residents

  • Young adults recovering from family breakdown or domestic abuse
  • Adults with mild mental health challenges
  • Care leavers who need additional life skills and confidence building

Support Approach

Residents receive 3–4 structured sessions per week with a dedicated keyworker. These sessions focus on strengthening personal routines, emotional wellbeing, and goal setting.

Advisors help residents create realistic plans around training, employment, and community integration.

High Support

Intensive Care and Risk Management

Tier 3 is designed for residents with high or complex personal needs that require consistent, hands-on support. This tier provides a safe, structured environment where individuals can rebuild trust, manage risk, and regain control.

Typical Residents

  • Ex-offenders re-entering the community
  • Individuals overcoming addiction or trauma
  • Adults with challenging behaviours or recurring mental health episodes

Support Approach

Residents receive daily structured contact, combining intensive keywork with coordinated multi-agency involvement (social care, probation, NHS, etc.).

Staff ensure a stable daily routine while addressing both practical and emotional barriers to recovery.

Tier 4

Complex Support

24-Hour, Specialist Care

Tier 4 is the most intensive level of support within the Homes2Care model — providing specialist, round-the-clock assistance for residents with significant or clinical needs. It focuses on safety, dignity, and long-term recovery.

Typical Residents

  • Individuals with severe mental health or neurological conditions
  • Adults requiring constant supervision or 1:1 staffing
  • Residents transitioning from institutional or hospital-based care

Support Approach

Tier 4 properties are staffed 24 hours a day, with dedicated 1:1 or 2:1 support teams working under clinical supervision.

Each care plan is bespoke, designed around medical input, psychological support, and daily living assistance.

Personal Advisors

The Role of Personal Advisors (PAs)

A Bridge Between Care and Independence

Every young adult within Homes2Care has access to a Personal Advisor (PA) — a trusted, consistent professional who helps them plan their future and navigate adult life.

For many care leavers and young people, the PA becomes the most stable adult relationship in their lives. They act as a mentor, advocate, and coach, guiding residents through housing, education, and personal challenges with compassion and consistency.
